Q: Were can I get application forms for the OA Founders Award?

A: Application forms for the OA Founders Award are currently sent out as part of the Lodge Program Pak, after a lodge has rechartered. You should contact your local Lodge Adviser or Lodge Staff Adviser to see if they have the ones sent to your lodge. If they can not find them, they can request replacements from the OA national office.
