PDF Format Information
The online versions of most documents on this web site are in
"Portable Document Format" (PDF). This allows a good
representation of the content and layout that
appears on the original printed version.
You need a special free program called the
"Adobe Acrobat Reader" to view and print a PDF format
document. Your system may
already have it installed, or you can download one for free
from the Adobe web site.
If you already have the Acrobat Reader installed on
your system, just select the document you wish to read
from the appropriate page, and your browser
should download the file and start the Acrobat Reader to view it.
Note: The documents on this site require the 5.0 version
or higher of the Adobe Acrobat Reader. If you have an earlier
version of the Acrobat Reader, you will not be able
to properly view these files. Some files on this site may be
viewable with older versions of Acrobat, but others will fail to
open or give error messages when accessed.

Fields in PDF Documents
Some forms on this web site contain "Fields" in the
PDF document.
This allows users to fill out most items on the downloaded form, then
print a completed version ready for signatures, etc.
To use the Field feature in a PDF document,
open the document in the Acrobat reader program
and select the "hand" tool. Then position the cursor inside a form field,
and click on the field. The cursor will change to a "I-beam" shape, and
allow you to type text in the field. The "Tab" and "Backtab" keys can be
used to move between fields. Once you have filled in all the appropriate
fields, you can use the Print button or
the "File" => "Print" menu command to print
the completed form. People who do not want to use PDF Fields can just
print the blank form and fill it in manually.
(Note: This PDF Field feature of the Acrobat reader program is only supported
on Macintosh and PC systems. Users of other platforms will have to print out
a blank form and fill it in manually.)
